The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone, or stop by in person to make an appointment!
Patient Forms
If you are a new patient to our office, we ask that you please arrive 10-15 minutes prior to your appointment, in order to ensure that you are able to fill out all of your new patient paperwork in time for your scheduled visit. We provide iPads on site, OR can send a link to your mobile device to provide flexible ways to fill out forms. We also have staff members who are eager to help at the point of any inconvenience/questions.
Insurance and Payment Options
We accept most insurance plans, but please contact our office to verify acceptance of your plan. We offer flexible payment plans and financing options like Care Credit. We accept cash, checks, and credit or debit cards.
First Visit What To Expect
Your first time at Maryland Family and General Dentistry will consist of an initial examination, and X-Rays. During the first visit, your diagnosis and recommended treatment plan will be explained.
What we need from you
- Your State ID and dental insurance card
- Referral slip (If applicable)
- Any recent dental X-rays
- List of medications you are currently taking
- Medical Clearance (if pregnant or have medical complications)
Scheduling
We strive to schedule your appointments in a timely manner. For dental emergencies, we will do our best to provide same-day appointments.
Cancellation
We value your time and we work hard to minimize any waiting room time for each patient when they come in. If you need to reschedule, we request that you kindly provide us with at least a 48-hour notice. The office reserves the right to charge a cancellation fee in the absence of a 48-hour notice.
